Resource Partner Survey

Directions to completing the survey:

1. Type directly into the light gray areas of the form. Use the TAB key to go to the next section.  Please fill out all information regarding your organization. 
   
   
2. The person's name you choose to list should be the contact person for you're organization and will be listed on the website.
   
   
3. Read and carefully check all “Services“ that your organization “Offers“ (Just click on the box to the left of the word “offered“ and an “X“ will appear)
   
   
4. Save the form as BHRNyourorganization. Return it to the Institute for Entrepreneurship by e-mail to dave.madej@cpcc.edu (preferred method) or fax at (704) 330-6852. You may also mail it to P.O. Box 35009, Charlotte, NC, 28235-5009.
   
   
5. Optional but requested: You may include your organization’s logo to appear on your page.  All logos can be no larger than 150x150 pixels (GIF or JPEG file)
